Online Fitness Class Waiver

Please read the following carefully. You are required to read and sign this Waiver prior to participating in any Flex Appeal classes/programs.
I am aware that any classes/programs offered by Flex Appeal Personal Training (either online or in-person) are designed to, and will challenge my cardiovascular, respiratory and musculoskeletal systems.
I am aware that as part of the online class/program, there will be warm-up and cool-down periods at the beginning and end of each session. I understand that these are necessary to help reduce the risk of injury and I accept full responsibility for any injury if I miss a warm-up or cool-down.
I confirm that I have either had a physical examination and/or have been given permission from my Physician to participate in such activities or that I have decided to participate in these classes/programs voluntarily and without the approval from my Physician and hereby accept all responsibility for my participation in any class/program offered by Flex Appeal Personal Training.
I agree to observe and obey all rules and warnings, and further agree to follow any written or oral instructions/directions given by the coach(es)/trainer(s).
I am aware of and agree to the use of photography and videography during any classes or programs (either online or in-person) for marketing purposes. This includes but is not limited to: Facebook, Instagram, Flex Appeal Personal Training website ( and any other online site/platform.
I understand there is an inherent risk associated with any exercise program/class provided by Flex Appeal Personal Training (either online or in-person) that could result in damage to personal or third party property, injury to myself or others, or death.
I agree to only attempt or execute moves/exercises that are within my physical capabilities, and I am aware that attempting anything beyond my capabilities could result in injury. I also understand that when participating in an online class/program, the teacher/coach cannot see my actions/movements and it is therefore my responsibility to ensure I am replicating the movements/actions as accurately as possible to maximise gain and minimise risk or injury.
I understand that High Intensity Interval Training also known as HIIT involves periods of intense exercise followed by low intensity recovery periods and will therefore consider my level and ability when participating in online HIIT sessions and choose an appropriate level of exercise and rest when i need to.
When participating in online HIIT sessions or online personal training sessions listen to your body:
  • STOP the exercise if you feel nauseous
  • STOP IMMEDIATELY if you experience chest pain or tightness
  • STOP the session if you experience severe joint pain

Physical Activity Readiness Questionnaire

Has your Doctor ever said you have a heart condition and recommended you only participate in medically-supervised physical activity?
Do you frequently suffer from chest pains during physical exercise?
Do you ever suffer from chest pains when not exercising?
Do you lose your balance due to dizziness or do you ever lose conciousness?
Do you have a bone, joint or any other health condition that causes you pain or limitations that must be addressed or factored in prior to undergoing an exercise program (e.g. Diabetes, Osteoperosis, High Blood Pressure, High Cholesterol, Anorexia, Epilepsy etc)?
Are you currently pregnant or have you given birth within the last 6 months?
Have you recently undergone surgery?
Do you take any medication, either prescribed or non-prescribed on a regular basis?
Do you have any injuries or Orthopaedic problems such as bursitis, bad knees, back, shoulders, wrist or neck?

Please Note: If you have answered YES to any of the questions above, you may be required to provide a Doctor's letter of consent.
